For professionals

For discharge planners, case managers and elder law attorneys.

When someone you're helping needs a place to live with care, or a plan to stay home, you can send them to me. Finding a place is free to the family.

What I do

Two things, in plain words.

  • Placement: I help an older adult and their family find a licensed place to live with care. Adult foster homes, assisted living, memory care. Free to the family.
  • Care navigation: when it isn't time to move, I write a care plan to stay home safely and stay on the family's team. Paid by the family.
How to send someone

Call or text. That's it.

Call or text (971) 610-0047. Give me the family contact's name and number, and the discharge date if there is one. The family can also call me themselves.

Please don't email or text me health details. I get the care needs from the family directly, with their OK.

Who I can help

Private pay and Medicaid, and the harder cases.

I look for places in every licensed setting in the Portland metro:

  • Adult foster homes, including homes that take Medicaid
  • Assisted living and residential care
  • Memory care
  • Independent living, for people who don't need hands-on care yet

I also take the harder cases: dementia with behaviors, two-person transfers, a mechanical lift, someone coming onto hospice. I'll look for places that can really do it, and I'll tell you plainly if I can't find one.

When the answer isn't a move, the family can ask me for a written care plan to stay home instead. That one is family-paid, and every price is on my Pricing page.

What you hear back

You're not left wondering.

With the family's permission, I'll let you know when we've talked, and where they ended up. Without it, I can't. I need the family's OK before I share anything about them.

How I'm paid

On a placement, the family pays nothing.

The community the family chooses pays me a referral fee. I tell the family which places pay me, in writing, before we tour anything. I don't take money from anyone else I send people to: no home care agency, attorney or mover.

If the right place doesn't pay me, that's still where I'll point them. My full disclosures are here.

A leave-behind

Print this page.

It prints with my number at the top, so you can hand it to a family or keep it at the nurses' station.

Have someone in mind?
