Who I am

Local Ties Senior Advisors is the trading name of Local Ties LLC, an independent long-term care referral and advisory practice in Portland, Oregon, operated by Tom McCahan. Local Ties is registered with the Oregon Department of Human Services as a long-term care referral agent, Certificate #2443, issued August 20, 2026 and valid through August 20, 2028. Registration does not constitute a recommendation or endorsement by ODHS.

How I am paid

For placement: when a client moves into a senior living community I referred them to, that community pays me a referral fee. This service is free to families. I tell every client which communities pay me before any tour.

For care navigation: families pay published fees, listed in full on the Pricing page. If a family I help with care navigation later moves into a community that pays me a referral fee, I am paid by both: the family for the planning, and the community for the referral. Either way, I tell you which communities pay me before any tour.

Referral practices

I do not refer only to communities that have contracts with me — if the best fit for a family is a community that pays me nothing, that is still where I will point them. I do not accept compensation from any other provider I refer clients to: no home care agency, attorney, mover, equipment vendor, or anyone else pays me anything.

I will never refer a client to a facility in which I or an immediate family member holds an ownership interest, and I will stop contacting anyone who asks me in writing to stop.

Your protections

Your placement information is shared only with your affirmative consent, each individual time. Oregon's licensed long-term care settings search — including complaint history for every licensed facility — is available to every family at ltclicensing.oregon.gov. To report concerns about a referral agent, contact the ODHS Long-Term Care Referral program.
